Secondary care means your healthcare provider has transferred your care to someone with expertise in your specific diagnosis.
Tertiary
Tertiary care is specialized care delivered in a hospital or similar care setting. It usually requires a referral from a primary or secondary care provider, and is typically only available at specialized medical centers. Tertiary care can include a range of procedures from cancer treatment to organ transplantation.
Continuing
Restorative
focuses on maximizing an optimal level of functioning, enabling clients to regain/retain their independence following the debilitating effects of illness or injury.
Continuity of care is an approach to ensure that the patient-centered care team is cooperatively involved in ongoing healthcare management toward a shared goal of high-quality medical care. Continuity of care promotes patient safety and assures quality of care over time.
